  • Provides custom alerts such as door opened at scheduled times of day.

Access Wander + Reader

Single Reader - Rythmos Wander AI™
  • For secure, tiered door access through resident Rythmos® wearables or staff fobs

  • Door will only open if device has proper credentials assigned to that to scan on reader

  • Options for both audible and LED alerts from reader in addition to centralized alerts to staff through Rythmos® CM app

Dual Access Wander + Readers

Dual Reader - Rythmos Wander AI™

Configurable, Advanced Wander Alert Options

Monitor Door With Keypad/Keycard

Reports access granted or denied attempts for each door.

Proximity Wander Alerts

When resident comes within certain range, it will trigger a wanderguard alert to staff.

Loitering Alerts

Allows staff to be notified when resident is loitering by “x” door within proximity. Distance and time is configurable.

Forced Door/Breached Alerts

Be alerted whenever door is forcibly opened without credentialed fob/key code.

Door Propped Alert

Be alerted whenever a door is open for a set duration of time (configurable).

Open/Closed Alerts

Be alerted whenever the door is an open or closed state.

Open Door Wander Alerts

Be alerted whenever door is open and a wander risk resident is within proximity.

Egress/Delayed Egress

Allows for normally locked doors to be released after holding lever for 15s in case of fire emergency.

Secure, Tiered Door Access

Ability to lock or unlock for staff or residents based on assigned access credentials for that door for their fob or wearable. Can also assign visitor fob access if/as needed.

Lockdown for non-credentialed fobs

If a non-credentialed resident or staff approach or tab reader, the door will stay locked down. If the door is always unlocked, the door will lock down and not allow access.

Automate Default Door Status

Can set any door to be normally unlocked, normally locked, or create a custom automated door schedule for specific lock or unlock times.

Wander AI™ for Data-Driven Solutions

Rythmos® Wander AI™ integrates data from several sources as part of our advanced solutions for elopement risk residents with centralized alerts and data for caregivers and staff. It’s more than door security: it’s about proactive, data-informed solutions.

  • Gathers real-time data from Kallos wearable on resident physical activity, detected falls, wander alerts, button press history, real-time, location data, and other uses.

  • Gathers real-time data from other Rythmos® sensors and wander devices in community to centralize and integrate resident behavioral insignts and other wander activities, like loitering by exit doors.

  • Ingests data from EHR records – such as ambulation, acuity, age, medical conditions, etc. – to enhance resident profiles and help customize care plans.

  • Integrates data on wander seeking behaviors and sleep quality from  unobtrusive sensors that can help flag changes in health, agitation, or other potential wander-seeking indicators.

  • Utilize real-time data insights and integration to improve and personalize care plans to meet any budget or need.

Wander FAQs

While caregivers can see incoming alerts -including fall alerts, wander alerts, and wearable button presses – through the Rythmos CM mobile app, only higher level admins and managers can see certain data and reporting through the Rythmos CM web portal. Our portal features real-time data and community mapping, historic data on resident activity, alert history, device usage and battery levels, ability to customize alert and device settings, and more!

For any questions about reporting and available data, reach out to our Rythmos support team (for existing clients) or schedule a free demo! We’d love to talk about how this data can be turned into action to improve senior care and wander solutions.

For any of our wander alert types (loitering, door open, egress, elopement, etc.), settings such as duration windows and proximity scanning distances can be customized based on need. Staff is notified of all alerts through the Rythmos CM portal and mobile app for response.

Our access + wander readers and some other sensors can be configured with audible alert sounds and/or LED light indicators depending on the device.

No, our patented location awareness for outside of the community tracking through LTE-GPS is only available for our Community + and Kallos wearables. All three of our wearable types, including our basic Community wearables, include in-community location monitoring accurate within 1-3 meters along with live community mapping to find residents through our Rythmos® CM mobile app for staff.

All Rythmos® wearables using the wrist wear style can add on a secure locking band that can only be unlocked using a magnetic key for use by caregivers or community staff. This helps ensure that residents at risk for elopement – especially memory care residents – will always have coverage when and where they need it.

Yes! Whether you use our Community or Community + wearables or our newest Kallos model, machine-learning fall detection is built as a baseline of the Rythmos® system along with other core features such as location awareness and nurse alerts all in a single device

For elopement risk residents using our secure locking band on their Rythmos® wearable, the wrist wear style is required.

For fall detection, there are two separate machine-learning algorithms we use for the watch vs. necklace wear style which factor in movement patterns and other differentiators. The algorithm for the necklace wear style is on average 20-30% more accurate when it comes to detecting falls, which is something to consider for seniors with advanced health conditions such as Parkinson’s.

In addition to the real-time location awareness from resident wearables, our smart sensors can be added with a resident’s room or senior community to add on additional wander management settings and alert types. For example, our presence sensors can track data whenever someone enters or exits a marked doorway and our window and door sensors can trigger alerts when a door is open or when a wander-risk resident is in proximity to an exit.

Other sensors are available to enhance fall-detection and other custom features to collect unobtrusive data and detect changes in behavioral patters that may signify a need for adapting resident care levels.
